I apply to donate my time in an effort to advance the mission and goals of We Own Mentor Empower, Nurture Each Other, Inc. (hereinafter referred to as W.O.M.E.N. E.O., Inc.). I further apply to participate in programs and activities sponsored by W.O.M.E.N. E.O., Inc. I understand that W.O.M.E.N. E.O., Inc. is a non-profit organization that performs services for charities, community organizations, and other non-profit groups, for which costs may be charged but which do not involve any profit or compensation to W.O.M.E.N. E.O., Inc.
I further acknowledge and agree to an understanding of the term “volunteer” and nature of the services I provide to W.O.M.E.N. E.O., Inc. as being a person who performs a service willingly and without pay.
I agree that W.O.M.E.N. E.O., Inc. and the organizations for whom it is performing services, their agents, officers, and directors shall not be liable for any injury that I may incur while participating, and I release and agree to hold harmless and defend them from any claim that I or anyone may have arising from my participation. I further assign to W.O.M.E.N. E.O., Inc. and consent to its unrestricted use and publication in any media of any photograph, recording, interview, videotape, or other recording of me in connection with any activities in which I may participate with W.O.M.E.N. E.O., Inc.
I agree not to initiate any claim against W.O.M.E.N. E.O., Inc. resulting from my service as a volunteer.
BY COMPLETING AND SUBMITTING THIS VOLUNTEER FORM TO W.O.M.E.N. E.O., Inc., I AFFIRM THAT I HAVE READ AND UNDERSTAND THIS AGREEMENT IN ITS ENTIRETY.
I FURTHER AFFIRM AND AGREE THAT THIS IS MY ELECTRONIC SIGNATURE AND UNDERSTAND THAT MY IP ADDRESS WILL BE RECORDED.
